An opportunity has arisen for 2 qualified Senior Practitioner Social Workers to join Community Health and Social Care Direct Team. This is a friendly and supportive team committed to person-centred practice which helps promote independence, wellbeing and choice.
Community Health and Social Care Direct Team provide the first point of contact for Adult Social Care across Newcastle upon Tyne. Your role will include; responding to crisis/emergency situations, triaging referrals, managing workflow, completing adult assessments and carers assessments, often in complex and challenging situations.
You will provide information, advice and support to prevent the need for long term support and promote independence. The role requires you to be flexible, adapt quickly and have the ability to make sound judgements, often in an emergency. You will also assist and support colleagues with their continuing professional development and mentor newly qualified and less experienced staff with some supervisory role. You will be committed to integration and have the ability to work in an MDT environment.
The role is fast paced and requires a high level of ability to undertake effective decision making with confidence and in liaison with other services. You must be able to use IT systems to accurately record actions and assessments and update file notes in a timely manner.
You must have a relevant social work qualification, such as Degree in Social Work, DipSW, CQSW, CSS or equivalent, and current HCPC registration.
Professional support, supervision and appraisal will be provided by the Team Manager of Community Health and Social Care Direct Team.
