We currently have a vacancy for a Health and Social Care Coordinator to work 18.5 hours per week Monday - Friday within Newcastle City Council’s Care Services teams, the post will be based at Byker Lodge, our short-term assessment centre for older people who are living with a cognitive impairment.
This post involves carrying out assessment and care planning work in order to achieve the best outcomes for people using the service, the post will involve some front-line work to establish people’s ability. We are looking for applicants with a flexible approach who are committed to making a real difference to people’s lives.
Our staff work closely with health colleagues and must be able to demonstrate a strong commitment to promoting independence, health, wellbeing, and choice to those who use our services. Maintaining the dignity of people must be always a priority.
